There are a concept and key components of methodology of the home care via digital technologies in the paper. Standard technical solutions and patients’ equipment sets for distant monitoring are proposed. Hardware and software solutions are still developing. They can be perspective for reduction of hospital stay length and emergency visits number without loss of medical care quality.
